Larry Leseberg, ValleyVet.com pharmacist Welcome to the Valley Vet Pharmacy. My name is Larry Leseberg, and I'm the Pharmacist in Charge who oversees this area of our business. The drugs in this category are restricted to use by or on the order of a veterinarian licensed in the United States. Feel free to email or call us about additional items not listed on these pages. Before we can process your order, we must have a prescription from your veterinarian on file. Three convenient ways to do that are:
  1. Let us contact your veterinarian for prescription authorization.
  2. Your veterinarian can contact us by fax/phone.
  3. You can mail us the original written prescription.
See below for details.
Let us contact your veterinarian: Provide us your veterinarian's phone and fax number allowing us to request prescription authorization from your veterinarian. When placing an online order you may provide this during Checkout on the Summary page. (Order will be placed on hold until we receive the prescription authorization.)
By fax from your veterinarian: Your veterinarian can fax the prescription from their office. The prescription fax form may be used by your Veterinarian to fax a prescription to us at 1-800-531-2390.

By phone from your veterinarian:
Have your veterinarian call in your prescription to us at 1-800-898-8026, 8 am - 5 pm Central Time, Monday-Friday. Your veterinarian may leave our pharmacist a voice message after hours.
By mail: You can mail the original written prescription to:

  How to Place an Order

If you would like to place an order before your veterinarian calls or faxes the prescription, you may do so. Our customer service representative can place the order on hold until we receive the prescription. As soon as the prescription is received, the order will be released.

NOTE: Prescriptions are refillable, if indicated, for one year from the date on which they were written. After that time, your veterinarian will need to authorize a new prescription.

Thank you for shopping with us.
